    Step 1: Register for Assessment

        The first step is to register for an assessment with My Aged Care. You can do this by calling My Aged Care on 1800 200 422 or by applying online

    Step 2: Your Assessment

        RING RING: You will then receive a call from a My Aged Care assessor to schedule an appointment for your face-to-face assessment.

        During this assessment your assessor will talk to you about your care needs.


        This assessment is to ascertain what level of assistance you need to support you living independently at home.


        Home Care Packages are appointed based on the complexity of your requirements and range between level 1 and level 4, level 4 is the highest level of care, support and funding you may receive.

    Step 3: Your Outcome

        You've got mail: You will receive a letter in the post following your assessment indicating which Home Care Package Level; you have been approved for.


        Waiting times for allocated funding to come through can range between 6 and 12 months, dependant on which level you have been allocated.


        If you are not eligible for a Home Care Package or whilst you are waiting for funded care Sunrise Home Care can still offer you their support and care via private Home Care.

    Step 4: Choosing a Provider

        Once you have received a letter in the mail from My Aged Care advising you that your Home Care Package has been “Assigned” the next step is to find a home care provider who’s right for you.


        To select Sunrise Home Care as your provider to complete your Home Care services for you, you can do so by calling us or by completing a form, we will then allocate you your own dedicated Home Care Package Manager who will handle the remainder of paper work on your behalf.

    Step 5: Tailor Made Care Plan

        Once you have selected Sunrise Home Care as your provider, your very own dedicated Home Care Package Manager will come to visit you in your home to spend time with you and tailor make a care plan specific to your care needs and requirements.


        You have complete control over your care plan and what care services you choose, our team are available to make recommendations and support you every step of the way.
